The submission requirements for this program are:

Completed ACORD applications for all requested lines of business

Current statement of values (preferably in Excel format)

Currently valued loss runs for the past five years (plus the current year)

Carrier required supplemental and activity-specific questionnaires

For fleet submissions, we can accept an Excel schedule as long as the required components are provided
Must-Have:

Adequate staffing & nurse staffing consistent with resident acuity

Minimum of 20 residents per community and 50 residents per account

3 years in operation or highly experienced management team
